La Fisioterapia Respiratoria nelle Bronchiectasie non-FC: perché e come
Le bronchiectasie non-fibrosi cistica (FC) costituiscono un’entità nosologica eterogenea, che si caratterizza per la presenza di tosse produttiva persistente, recidive di esacerbazioni infettive e pneumopatia ostruttiva.
I segni distintivi delle bronchiectasie sono la stasi delle secrezioni, spesso infette, la ridotta clearance del muco e la dilatazione delle pareti delle vie aeree, con ispessimento, distruzione e perdita dell’integrità strutturale.
I metodi di trattamento che favoriscono la clearance del muco sono considerati essenziali per migliorare la funzione respiratoria e ridurre la progressione della malattia polmonare ed hanno un impatto positivo sulla qualità della vita dei pazienti.
Nonostante la mancanza di evidenze scientifiche di alta qualità, dovuta alla scarsità di studi nella popolazione pediatrica, sono ampiamente prescritti nella pratica clinica quotidiana. Nessun singolo tipo di tecnica è superiore ad un altro e la prescrizione del tipo di fisioterapia nei bambini con bronchiectasie è variabile. È pertanto opportuno conoscerne i principi generali in maniera da poter prescrivere la tecnica migliore in relazione all’età e alle attitudini di ogni singolo bambino.
Non-CF bronchiectasis is a chronic condition of the airways characterized by the presence of persistent productive cough, recurrent infectious exacerbations and obstructive pulmonary disease. The hallmarks of bronchiectasis are stasis of infected secretions, reduced mucus clearance and bronchial dilatation with thickening, destruction and loss of airway structural integrity. Methods that improve muco-ciliary clearance (airway clearance techniques, ACTs) are considered essential to optimize respiratory function, reduce the progression of the disease and improve quality of life.
Despite the lack of high-quality scientific evidence due to a few numbers of studies in pediatric population, ACTs are widely used in clinical practice.
No single type of technique is recognized to be superior to another one and the prescription of the type of physiotherapy in children with bronchiectasis is variable. It is therefore advisable to know its general principles to be able to prescribe the best technique in relation to patient’s age and attitudes.
